What Do I Need to Register my Child
- Registration Form (you can pick it up during Round-Up or during school hours)
- Pre-School Questionnaire Form (part of the registration packet)
- Forest Grove School District Proof of Residency Form (part of the registration packet)
- Bus Registration Form (if you live a mile or more from school)
- Please provide the following documents for copying:
- Student's Birth Certificate
- Student must be five years old by September 1, 2024
- Updated Immunization records
- Proof of residency:
- An item from Category 1
- Real Estate Tax Bill from Current Calendar Year
- Signed Rental or Lease Agreement & Proof of Payment
- Mortgage Statement
- Notarized statement from owner/renter indicating:
- Names of those living with the owner/renter
- Anticipated length off time of residence with owner/renter
- Note - Owner/renter proof must be documented
- An item from Category 2
- Gas/Electric Bill
- Water Bill/Trash Bill
- Phone Bill (no cell phone)
- Cable Bill
- Vehicle Registration
- Voter Registration Card
- Voter Registration Card
- Driver's License
- Current Public Aid Card
- Homeowner/Rental Insurance Policy & Payment
- An item from Category 1
- Student's Birth Certificate
